Data Loss Source: Express Scripts, a pharmacy benefits manager, received a letter in October threatening to expose millions of patient records. The letter asked for money and included client data.

Geographic Focus: United States
Data contained: Names, dates of birth, social security numbers and in some cases prescription information for 75 people.
Additional Notes: Express Scripts is offering $1 million reward for information leading to the data breach. Anyone with information on the breach should call the FBI at 800-CALL-FBI.

Data Loss Source: Arizona’s Department of Economic Security said five hard drives stolen from a storage unit contained the personal information of up to 40,000 children.

Data contained: Social Security numbers, names, addresses and phone numbers.
Additional Notes: A department spokesman said the hard drives were all password-protected and the agency does not believe any of the information has been accessed.

Data Loss Source: The University of Florida said that an unknown attacker gained access to a server in its College of Dentistry

Geographic Focus: Florida
Data contained: Personal information on the compromised server included names, addresses, birth dates, Social Security numbers, and in some cases, dental procedure care information for patients dating to 1990.
Additional Notes: The university has contacted the FBI and is investigating the security breach.

Data contained: Social Security numbers, salary information, home addresses and other data.
Additional Notes: Victims included 700 members of International Union of Operating Engineers Local 609, which represents custodial, nutritional services, security- and alarm-monitoring workers.
