Data Loss Source: Hundreds of Ground Zero workers were exposed to potential identity theft when stacks of payroll sheets – which included their names and Social Security numbers – were dumped in the trash along with confidential plans for the new World Trade Center.

Data Loss Source: A Rhode Island Department of Administration computer disk containing the social security numbers of nearly 1,400 people has been reported missing.

Additional Notes: The state Department of Administration says there’s no evidence that any of the numbers have been misused or that the disk fell into the hands of an unauthorized person.

Data Loss Source: The state of Pennsylvania was forced to pull the plug on a voter registration Web site after it was found to be exposing sensitive data about voters. Because of a Web programming error, the Web site was allowing anyone on the Internet to view data.

Data Loss Source: A Binghamton University employee mistakenly sent an email attachment containing the names, grade point averages and Social Security numbers of junior and senior accounting students to another group of School of Management students.

Additional Notes: The e-mail, sent by the Coordinator of Undergraduate Advising for the School of Management Brian Perry, was supposed to go to accounting faculty members seeking input on student awards. It also should not have contained “unnecessary information,” the university told students in a letter informing them of the situation.
