Additional Notes: Police are urging anyone who has been a customer of Crosslines to contact their bank and the three credit bureaus, as well as be on the lookout for fraud.

Data Loss Source: A computer containing personal information on Systematic Automation Inc/Los Angeles DWP water and power employees was stolen from an outside vendor.

Data contained: A The data that was taken on active DWP employees included names, Social Security numbers, dates of birth, employee identification numbers, salaries, work locations, deferred compensation balances (but not account numbers), insurance plan coverage and health care benefits selection.
Additional Notes: The utility will pay for a credit monitoring service for each of its 8,275 workers.

Long Island University Students at Risk

Data Loss Source: Long Island University has sent letters to 25,000 to 30,000 students informing them that tax forms mailed to them last week in “defective mailers” might have led to identity theft, and recommended that students put fraud alerts on their credit files.

Data contained: A 1098-T tuition statement, which has to be sent to every student who paid tuition in the 2007 calendar year and postmarked by Jan. 31, contains the student’s name, address and Social Security number.
Additional Notes: The mailers containing each student’s annual 1098-T “Tuition Statement” were supposed to have adhesive on all four sides. But one side of each envelope was missing adhesive, according to LIU officials, which caused about half of the statements to be damaged by U.S. Postal Service processing machinery.

Data contained: The theft possibly exposed names, addresses and Social Security numbers.
Additional Notes: All diocese computers are password protected. The diocese is urging current and former employees who might have been affected to put a fraud alert on their credit reports.
